Why complete the lessons learnt report?

This report helps capture what’s been learned during your SHDF Wave 2 project – what worked well, what didn’t, and what might help others in the future. 

Your report helps: 

  • Improve future SHDF waves with better guidance and fewer delivery barriers 
  • Share good practice across the sector 
  • Support your own organisation in refining delivery and avoiding repeated issues 

You don’t need to submit long paragraphs. Bullet points and brief reflections are fine. Successes are just as important as challenges. 

How to complete the report

•    Use the reflection prompts to help you think through each section  
•    Refer to your Lessons Learnt Log - it contains much of the content already 
•    Keep answers clear, practical and specific. Use real examples where possible (dates, numbers, actions). 
•    Delete grey guidance boxes in the template before sending.
 

Quick checklist

    1. Added your project details and named a key contact 
    1. Written a 150-word summary of your project and lessons 
    1. Reflected on each of the themed lessons 
    1. Completed the forward-looking questions 
    1. Shared any counter-fraud insights and added your confidence rating
